Do You Need a Summer Physical?

The Dr. Robert E. Appleby School Based Health Center is pleased to announce that we will once again be available for summer physicals. Whether it is a sports, 10th grade (required for all 10th graders) or college physical we can help.  Please be aware that Norwalk Public Schools requires that students playing fall sports, MUST have a physical dated JULY 1st or after.

College bound seniors should note, that most colleges will require you to have a meningitis vaccine if you are living in a dormitory.  We will be happy to provide this vaccine for our college bound seniors.

The Dr. Robert E. Appleby School Based Health Center will conduct all physicals at Brien McMahon High School (because of NHS construction activities) from July 23-27th and July 30- August 3rd from 8:30 AM to 12:30 PM.  You MUST make an appointment to schedule a physical or vaccine.  To make an appointment, call 838-4481 x254 and speak to or leave a message for Dale Ford.  This is our Norwalk HS School Based Health Center number which we are using for bookings for students at both schools.
