PARTICIPATION REQUIREMENTS

There are a number of requirements for students to participate in the program. These include a number of forms. Links to these forms can be found below.

The Norwalk Public Schools Athletic Handbook with all the information on the program can be accessed by clicking here.

REQUIRED FORMS

Only ORIGINAL physical forms, not copies or fax sheets, will be accepted.

Parents (or guardian) must complete/sign the following forms. A complete packet of forms can be printed by clicking here. If you need individual forms they can be found by clicking on the following links (click on name to print copy):

  1. Parent/Guardian Agreement (Drug and Alcohol Form)
  2. Permission to Participate in Interscholastic Sports
  3. Sports Participation Health Record
  4. Medical Examination Form
  5. Emergency Medical Treatment Form
  6. Parent/Guardian Agreement Regarding Performance Enhancing Drugs

Athletes must complete/sign the following forms (sign copies of above):

  1. Athlete Pledge (Drug and Alcohol Form)
  2. Permission to Participate in Interscholastic Sports
  3. Medical Examination Form
  4. Athlete Pledge Regarding Performance Enhancing Drugs

Your physician (M.D.) must complete/sign the following form (same as form above):

  1. Medical Examination Form (must be completed after July 1 and prior to the beginning of the athletic season)

All forms must be returned to the coach before an athlete can practice or play.
